Annual Exam QVS will reimburse up to $120 dollars each year for vision exams.
Glasses or Contact Lenses QVS will reimburse up to $200 for contact lenses or glasses every two years. This benefit may be used incrementally (e.g., $100 per year), or as a lump sum over two years.

QVS provides group health plan benefits for its employees. Following one month of employment with QVS, an employee who is contracted to work at least 20 hours per week is eligible for health benefits, prorated based on the amount of hours worked.
QVS presently pays the full amount of insurance premiums for all eligible full-time employees.

If you work at least half-time for QVS, you could choose to receive a healthcare bonus instead of health insurance. This bonus is intended to help offset the cost of receiving health insurance from another source.
If you do choose the bonus, we need something to show that you’re receiving health insurance from another source. This could be a bill, an insurance card, or whatever makes the most sense for your situation.
After demonstrating eligibility, an eligible full time employee is paid an additional $230.77 bi-weekly through the QVS payroll. A qualifying part-time employee is paid an additional $115.38 bi-weekly. These additional payments are subject to applicable tax withholding. The bonus amount ceases at any termination of employment and is not provided during any unpaid leave or period of suspension.

Although QVS doesn’t make employer contributions toward your retirement, we will pay enrollment fees so you can setup a 403(b) retirement plan. You can designate some portion of your pre-tax income to go into your 403(b). For more information, please talk to the operations and admin manager.

Because access to email and video conferences are necessary to your job performance, QVS offers financial support for maintaining a functional home office. To repair or upgrade equipment, employees may request reimbursement up to $X every two years. A new employee may request up to $Y to setup their office for the first time.
